This form must be submitted when you have completed the requirement of 40 continuing education units (CEUs) over a four-year period.

The CEU completion certificates are what you receive as proof that you have completed your CEU(s). For example, if you are seeking CEUs from responding to MHTP-approved books, recordings, or podcasts, you will receive a completion certificate after you follow the instructions for submitting that response. Do not upload your book response here; only upload the completion certificate after the reviewer approves your response.
Partial documentation for fewer than the total required 40 hours will not be accepted or kept on file.
